Adult Day Health Programs
Non-residential, community-based program that provides health-related support services in structured adult group setting
Ambulatory Surgery Center
Non-hospital entity that provides surgical services without overnight stay
Clinic
Includes community health, outpatient clinic, urgent care, and mental health centers, and some physician practices
Dialysis Center
Non-hospital facility that provides maintenance dialysis to patients with chronic renal disease
Emergency Medical Services
EMS first response service or ambulance service licensed by DPH
Home Health Agency
Entity that provides health and/or homemaker services to individuals in their residences
Hospice
Hospice program or free-standing hospice facility
Hospital
Acute or non-acute health care facility providing treatment to admitted patients; licensed as a hospital by DPH
Long-Term Care Facility
Nursing, convalescent, rest home, etc.
Medical or Dental Office
Individual practice, not licensed as a clinic or owned by a hospital or other licensed health care entity
Mental Health
Psychiatric hospital, psychiatric unit within a hospital, residential or community-based mental health program licensed by DMH; doesn't include mental health clinics
Pharmacy

Substance Use Disorder Treatment Program
Program licensed by the Bureau of Substance Abuse Services; doesn’t include programs operated by hospitals or mental health facilities
