This content is relevant to licensed hospice programs and free-standing hospice facilities. Note that safety-related reporting requirements imposed by the Centers for Medicare & Medicaid Services (CMS), MassHealth, or accrediting organizations are not included in the information provided here. Click here to learn more about what is and is not included in the Navigator.

Adverse Event Categories

Event Type Description
Abuse or Neglect Suspected patient abuse, neglect, mistreatment or misappropriation of property
Criminal Suspected/potential criminal activity (e.g. sexual assault, impersonation)
Device Medical device failure, malfunction, mislabeling, user error, etc.
Facility Emergency Fires, labor actions, evacuations, etc.
Other Choose this category if your event does not appear to fit elsewhere.
