Below, you’ll find patient safety events grouped into categories. Click on the most relevant category to see descriptions of events that need to be reported by facilities and programs licensed by, contracted with, or operated by the Department of Mental Health (DMH).

Psychiatric hospitals/hospital units dually-licensed by the Department of Public Health may find additional reporting requirements by visiting the Hospital section as well. Information for mental health clinics licensed by the Department of Public Health can be found in the Clinic section. See the glossary for more complete definitions of facilities and programs.

Note that safety-related reporting to the Centers for Medicare & Medicaid Services (CMS), MassHealth, or accrediting organizations is not included in the information provided here. Click here to learn more about what is and is not included in the Navigator.


Reportable to DMH

Event Type Description
Incidents Incidents that involve a death, felony, assault, abuse or other serious events for investigation by the department
Adverse Drug Event - Substance Use Treatment Services Adverse drug events that occured in mental health facilities providing certain substance use disorder treatment or services

Event Type Description
CMS and MassHealth Broad overview and links to resources related to conditions of coverage and participation in these programs
Communicable Disease or Unusual Illness Communicable disease, unusual illness, or other health condition of public concern
Drug Diversion Loss or theft of controlled substances
