Information regarding reporting regulations for programs licensed by the Department of Mental Health and offered at clinics may be found in the Mental Health section of the Navigator. Blood | Collection, administration, storage, handling, etc. of blood products |
Criminal | Suspected/potential criminal activity (e.g. sexual assault, impersonation) |
Device | Medical device failure, malfunction, mislabeling, user error, etc. |
Facility Emergency | Fires, labor actions, evacuations, etc. |
