How to report Serious Incidents to the Office of Emergency Medical Services

1. Within 7 days, e-mail a serious incident report form to OEMS.SIR@state.ma.us.

2. Within 30 days of filing the initial report, file an 1) investigation report of the incident and a 2) plan of correction and preventability with the Office of Emergency Medical Services (OEMS).

Tips:

Your local government (e.g. city, town) may require that you submit a copy of the report to them and/or may have additional reporting requirements.
