How to report | If you have had a disciplinary action taken against any of your professional licenses issued by any jurisdiction, you must report by mail the final decision, consent agreement or other such document issued by the licensing authority to your board's administrative address. Additionally, as part of the license renewal process, your are required to attest that you have reported to your board all disciplinary actions taken against your license and convictions. |
