How adult day health programs report Serious Incidents to the Department of Public Health

1. Immediately

If the SRE involved any of the following, call DPH. (See contact information below.)

  • Unanticipated death*
  • Evacuation of the facility
  • Fire
  • Suicide
  • A serious criminal act
  • Pending/actual strike

*Death that is unanticipated, unrelated to the natural course of the participant’s illness or underlying condition, or that is the result of an error or other incident.

If the SRE involved a reportable illness**, immediately submit an electronic report using the Virtual Gateway - Healthcare Facility Reporting System (HCFRS).

**Reportable illnesses are those believed to be part of a cluster or outbreak, believed to be unusual, related to food consumption, or believed to be transmissible through food.

If the incident did not involve any of these, go directly to step 2.

2. Within 7 Days

Submit an electronic report using the Virtual Gateway - Healthcare Facility Reporting System (HCFRS)

Tips:

  • If the incident also involved suspected abuse, neglect, or mistreatment of a participant, or misappropriation of a participant’s personal property, this should be immediately reported to DPH.
