How hospices report Serious Incidents to the Department of Public Health

1. Call DPH at 617-753-8150 if the

incident involved any of the following, and then complete step 2:

  • Fire
  • A serious criminal act
  • Pending/actual strike

If the incident did not involve any of the of the above, go directly to step 2.

2. Submit an electronic report using the Virtual Gateway | Healthcare FacilityReporting System (HCFRS).
