How to report adverse drug events to the Department of Mental Health

Note: These reporting instructions are applicable to any facility licensed, contracted, or operated by the Department of Mental Health (DMH) that offers “substance use disorder treatment services.”

Report the adverse drug event to the DMH Director of Licensing at 617-626-8167, no later than the next business day after the event occurred.
